RE: Appeal of claim denial
Patient: Lucas Solis
Date of birth: April 9, 1981
Member ID: M7731904
Claim number: FW-48213077
Date of service: February 11, 2025
Procedure: D2740, crown, tooth 19
Amount billed: $1,240.00
Amount paid: $0.00
Dear Ms. Wen,
We are appealing the denial of the claim above and ask that the plan review it again and reconsider payment for the crown on tooth 19.
Your letter dated March 3, 2025 states the reason for denial as "service not necessary based on submitted information." We believe the clinical records show why Dr. Luke Vance recommended this treatment, and we are sending them with this letter.
Clinical summary from Dr. Vance:
Mr. Solis reported cold sensitivity on the lower left for about two months. On examination, tooth 19 had a large, old MOD amalgam filling and a fractured distal cusp. The bitewing taken on February 11, 2025 shows recurrent decay under the existing filling, and the decay was confirmed when the old filling was removed. Dr. Vance found there was not enough sound tooth structure left to support a new filling, so he recommended a crown.
We ask that the plan reverse the denial and process the claim for procedure D2740 on tooth 19. If the plan needs anything else to complete its review, please call Lucille Tate at 336-555-0183.
Enclosures:
- Bitewing x-ray dated February 11, 2025
- Intraoral photo of the fractured distal cusp on tooth 19
- Clinical notes from Dr. Vance for the February 11, 2025 visit
Sincerely,
Dr. Luke Vance

Deadline note: The denial letter gives 180 days to appeal. Counting from the March 3, 2025 denial date, that is August 30, 2025. Please confirm the exact deadline against the denial letter, and we recommend sending this well before then.
Questions for the owner
- What is the state and ZIP code for both the office address and the plan's appeals address?
- Does the plan require a signed patient authorization or a specific appeals form with the letter?
- What is the dentist's full credential line (for example license or NPI number) to add under his signature, if the plan asks for it?
- Does the plan accept appeals by fax or portal, and if so, what is the number or web address?
What it does
Turns a denied or underpaid dental claim, the denial reason, and the dentist's clinical notes into a clear, professional appeal letter. It also lists the records to enclose and flags anything still missing.
Use it when: Use when a dental office needs to write an appeal letter for an insurance claim that was denied or underpaid, such as 'write an appeal for this denial,' 'the insurance denied the crown,' or 'fight this claim denial.'
