Behavior Intake Form to Training Plan Recommendation
Reads a new client's behavior intake form and turns it into a clear, organized training plan recommendation the trainer can review and send.
Before and after
Hi Irina,
Thank you for filling out the form for Pickle. Here is what we read and what we recommend.
About Pickle
Pickle is a 2 year old rat terrier mix with a brindle coat and one blue eye. You adopted her from a rescue 8 months ago, and she lives with you and Ian Sears. There are no children or other pets at home. She started a puppy class at a pet store but did not finish it.
Safety concern, recommended now
- Snapping at the vet tech: You wrote that Pickle snapped when the tech tried to hold her paws. We recommend we talk this through on our call before her first session so we can plan how to handle touch and handling safely. We also recommend mentioning it to your veterinarian, since pain can sometimes be part of a reaction like this, and a vet is the right person to check. As the trainer, I will decide after we talk whether Pickle is a fit for the program.
Recommended now
- Pulling on leash: You described it as "pulling like crazy." We recommend working on loose-leash walking.
- Not coming when called outside: We recommend building a reliable recall, since this affects her safety outdoors.
- Chewing the couch when left alone: We recommend working on how Pickle handles time alone, along with giving her appropriate things to chew.
- Nonstop barking at the window: We recommend working on how she responds to people passing by.
Also recommended
- Jumping on guests: We recommend teaching Pickle a calm greeting.
Recommended program
We recommend our 4 week Foundations program, with one private 60 minute session each week. We also recommend adding our Leash Manners group class for the pulling.
Starting home tasks
- Keep Pickle on a leash indoors when guests arrive, so she cannot jump on them.
- Limit her view of the window while you work on the barking, for example by closing the blinds.
- Do not leave her alone with the couch for now.
- Do not force her to have her paws handled until we have talked.

Questions for the owner
- Did the vet tech report any injury, and has Pickle's paw or any other area been checked by a vet?
- When did the snapping happen, and has she done anything similar at home or with other people?
- Which days and times can Irina and Ian attend sessions?
What it does
Reads a new client's behavior intake form and turns it into a clear, organized training plan recommendation the trainer can review and send. It uses only what the client wrote and what the trainer says about the business's programs.
Use it when: Use when a dog training business needs to turn a client's behavior intake form into a training plan recommendation, such as: "read this intake form and suggest a plan," "what program should this dog start with," or "write up a plan for this new client."
