Puppy Vaccine Record Check Before Class
Compares a puppy's vaccine records against your written class requirements and tells you what is on file, what is missing, and what is unclear.
Before and after
Requirements checklist (from your class rules)
- At least 2 rounds of DHPP
- First DHPP at least 14 days before the first class (June 14)
- Rabies shot if the puppy is 16 weeks or older at the first class
Record check
- DHPP round 1: April 19. Met. April 19 to June 14 is 56 days, which is more than 14.
- DHPP round 2: May 10. Met.
- Rabies: Not on file. Whether it is required depends on Gubbins' age on June 14. You said about 14 weeks now, and the birth date is not on the records, so I can't confirm the age on June 14. Marked Unclear.
- Bordetella (May 10): on file. Your rules did not list it, so it is not required.
Paperwork notes
- Puppy name (Gubbins) is on the records.
- Clinic name (Pinegate Valley Vet) is shown, with a stamp.
- No vet signature that you can see.
- No birth date on the records.
Result: Needs your review
The DHPP requirements are met. The rabies requirement can't be confirmed until you know Gubbins' exact age on June 14. The missing signature is up to you to accept or ask about.
Draft message to Margaret Sutton
Hi Margaret,
Thanks for sending Gubbins' records from Pinegate Valley Vet. Both DHPP shots are on file and meet our timing rule for Puppy Foundations starting Saturday, June 14.
Two quick things so we can finish your enrollment:
- Please send Gubbins' birth date. Our class rules ask for a rabies shot if a puppy is 16 weeks or older at the first class, and we need the date to see if that applies.
- If you can, ask the clinic for a copy of the records with a vet signature on it.
You can reply here or call us at 319-555-0122. We're looking forward to meeting Gubbins.

Questions for the owner
- Do you accept stamped records without a vet signature?
- What is the best way for owners to send records to you (email, text, or in person)? The message says "reply here," so confirm that works.
What it does
Compares a puppy's vaccine records against your written class requirements and tells you what is on file, what is missing, and what is unclear. It then drafts a short message to the owner so the puppy can be enrolled or the owner knows what to send.
Use it when: Use when a dog training business needs to check a puppy's vaccine records before class enrollment, such as: check this puppy's shots, are these vaccine records good for class, review vaccine paperwork, or write the customer about missing records.
