Data Backup and Data-Loss Notice
Ivymoor Phone and Computer Repair
6116 Harrowmoor St, Aspenglen
319-555-0146
Customer: Susan Wagner, 319-555-0147
Device: Zephyrbook 14 laptop, silver, cracked hinge cover, pelican sticker on the lid
Repair: Replace the failing hard drive with a new SSD and reinstall Windows
What this repair means for your data
Our technician, Tanya Alston, found that the hard drive is failing and is clicking. Replacing the drive and reinstalling Windows means everything on the old drive could be lost. Because the drive is clicking, a backup may not copy everything.
Backup
We recommend a backup before this repair. We back up to an external drive. The fee is $45 and it takes about 2 hours. We keep the copy for 30 days, then wipe it.

Data-loss acknowledgment
I understand that data on this device may be lost or unreachable during this repair. I understand that Ivymoor Phone and Computer Repair is not responsible for lost data, and that the shop will make a reasonable try at the backup. Because the drive is clicking, I understand the backup may not copy everything.

Questions for the owner
- What do you want to tell the customer if the backup cannot copy some files, such as a follow-up call or a note on the invoice?
- Do you want a passcode or sign-in policy added to this notice?
- Please have a lawyer review this waiver wording, and confirm locally that it holds up before you use it.
