Progress update for Gwen Pennington
Date: April 15
Therapist: Felix Mueller
Hi Gwen,
You started with us on March 4 after your left knee replacement in January, and you were referred by Dr. Henrik Price. It has been about six weeks, and here is where you stand compared to the goals you and Felix set.
Goal 1: Bend your knee to 110 degrees
- Started at 78 degrees, now at 104 degrees, a gain of 26 degrees.
- You are 6 degrees away from your goal. This one is improving.
Goal 2: Walk 20 minutes without stopping
- Started at 6 minutes, now at 15 minutes, a gain of 9 minutes.
- You are 5 minutes away from your goal. This one is improving.
- Your pain while walking started at 6 out of 10 and is now 3 out of 10, a drop of 3 points.
Goal 3: Go up and down stairs with one rail
- At the start you needed both rails and a break at the landing. Now you use one rail, but you still pause on the bottom steps.
- This one is improving, and we will keep working on it.
Attendance and home exercises
You have come to 10 of 12 scheduled visits. You told us you are doing your home exercises most days, and we encourage you to keep that going.
What we recommend next
We recommend 2 visits a week for 4 more weeks.
If you have questions, call us at 234-555-0178. We are at 8665 Redford St, Southcrest.

Questions for the owner
- Do you want the update to include a plan for the stairs goal or home exercise changes? Felix's notes did not list any.
- Should the update mention a date for the next re-measurement? None was given.
What it does
Turns the therapist's visit notes, measurements, and goals into a clear progress update that shows where the patient started, where they are now, and what the clinic recommends next. It reports what the notes say and does not add clinical judgment of its own.
Use it when: Use when a physical therapy clinic needs to turn visit notes, measurements, and goals into a clear progress update for a patient, a referring doctor, or an insurer contact, such as: write a progress update, summarize how this patient is doing, or write up where they are compared to their goals.
