Before and after
Subject: Gnocchi's visit summary
Hi Mr. Miles,
Thank you for bringing Gnocchi to Ridgecrest Animal Hospital today. Here is a summary of his visit so you have everything in one place.
Why Gnocchi came in
Gnocchi had been limping on his right back leg for 3 days and yelping when he jumped off the couch.
What Dr. Alma Landry found
- Gnocchi's right knee was painful and had mild swelling.
- His kneecap sits where it should, and the test for a torn knee ligament (the drawer sign) was negative.
- The X-ray of his right knee showed no broken bones and a small amount of extra fluid in the joint.
- Dr. Landry's assessment is a soft tissue sprain.
What we did today
- Examined Gnocchi and took an X-ray of his right knee
- Gave a meloxicam injection in the clinic
- Trimmed his nails at no charge
Medications to give at home
- Meloxicam liquid (1.5 mg/mL): 0.4 mL by mouth once a day with food for 5 days. Start tomorrow morning.
Care at home
- Strict rest for 2 weeks.
- Leash walks only for potty breaks, 10 minutes at most.
- No stairs and no jumping for 2 weeks.
- Use an ice pack on the knee for 10 minutes, twice a day, for the first 3 days.
Call us or seek emergency care if

Questions for the owner
- Do you want to add the date and time of the 2 week recheck once it is booked?
- Should the summary list a separate clinic number for after-hours emergencies?
What it does

Use it when: Use when a veterinary clinic needs to turn exam notes, treatment records, or discharge details into a clear visit summary for the pet owner, such as: write up the visit for the owner, send home instructions, or explain what we found today.
